Which Formulary Should I Choose?

The information below will help you to identify which formulary is applicable to you. However, the best way to access the correct formulary is through MyChart or by entering your member number or group number on the Formulary Lookup.

Note: Most of Unity Health Insurance's members use one of the two Standard formularies.

Standard Choice

For members whose pharmacy benefits are covered by a fixed copay on a three- or four-tier plan and are part of the Commercial HMO /  POS network.


For members with HSA plans. This formulary features a one- or two-tier Pharmacy Benefit in the Commerical HMO / POS network.

Metal Choice

This formulary serves members who purchased a metal tier (Gold, Silver or Bronze) plan that features fixed copays for prescriptions.


This formulary serves members who purchased a Gold, Silver, Bronze or Catastrophic plan with pharmacy benefits that are subject to deductible and coinsurance and have no fixed copays.
